St. John’s Receives PCMH Recognition

St. John’s Medical Group Primary Care Offices are proud to announce their reception of Level 3 Patient-Centered Medical Home Recognition by the U.S. Department of Health & Human Services. These offices are located at 495 Beach 20th Street, Far Rockaway, 431 Beach 129th Street in Belle Harbor, and 275 Rockaway Turnpike in Lawrence.
The Patient-Centered Medical Home (PCMH) designation recognizes physician practices that focus on care coordination, building care relationships, and encouraging patients and families to participate in self-management. One in six primary care physicians in America practices in a NCQA- Recognized practice. Under this care model, practices place emphases on partnership, communication, access to care, patient focus, self-management, care coordination, community services, technology, e-prescribing, and patient registries.
Patients at a PCMH are expected to take accountability for their own health care by staying informed about their health and participating in managing their own preventative care and chronic care. Through this partnership between patient and physician, the patient receives improved access to high-quality care, as well as focus on the patient as an individual and a partner in their health care.
