St. John’s Episcopal Hospital Achieves Age-Friendly Health System Level 2 Designation

Far Rockaway, New York, October 24, 2023 – St. John’s Episcopal Hospital is proud to announce that it has achieved a Level 2 designation as an Age-Friendly Health System by the Institute for Healthcare Improvement (IHI). This significant designation certifies St. John’s as Committed to Care Excellence and demonstrates our unwavering dedication to providing high-quality, evidence-based care specifically tailored to the needs of older adults.
Over the past three months, St. John’s has diligently collected data, focusing on the quality of care provided to older adults through the implementation of the 4Ms framework: What Matters, Medication, Mentation, and Mobility. This structured approach ensures that each patient receives comprehensive, personalized care, considering every aspect of their health and well-being.

As of October 2023, St. John’s Episcopal Hospital stands alongside over 1,900 esteemed healthcare providers, including hospitals, practices, care clinics, and nursing homes, all of which have been recognized as Level 2 Age-Friendly Health Systems. Collectively, this global movement has positively impacted the lives of more than 2,480,000 older adults through the consistent practice of the 4Ms care approach.
Age-Friendly Health Systems is a commendable initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), carried out in collaboration with the American Hospital Association (AHA) and the Catholic Health Association of the US (CHA).
