Episcopal Health Services Launches Community Health Programs to Address Diabetes and Hypertension

Far Rockaway, New York, September 18, 2024 — As diabetes and hypertension continue to have a significant impact on the overall health of residents on the Rockaway Peninsula, Five Towns and surrounding communities, Episcopal Health Services (EHS) is stepping up to help combat these critical issues through vital programs aimed at prevention and management.

EHS recently launched its National Diabetes Prevention Program, a year-long initiative backed by the Centers for Disease Control and Prevention (CDC) to help individuals at risk for Type 2 Diabetes. Led by the EHS Population Health Team, the program empowers participants to make lifestyle changes to prevent or delay the disease. Participants benefit from weight loss, improved health, reduced risk of diabetes and support from peers and trained lifestyle coaches.

“This evidence-based program focuses on behavior modification to help participants achieve and maintain weight loss. By preventing type 2 diabetes, we’re not only improving health outcomes but also reducing healthcare costs and fostering healthier, more resilient communities,” said Preet Kukreja, MBA, MHA, FAPM, Director of Population Health at Episcopal Health Services.

In addition to addressing diabetes, EHS is offering the National Healthy Heart Ambassador program. The four-month, CDC-endorsed initiative is designed to encourage individuals with hypertension to effectively manage their condition. Through monthly workshops led by a social worker, a registered nurse and dietician, participants receive personalized guidance, nutrition education and self-monitoring techniques to control their blood pressure.

“The Healthy Heart Ambassador Program serves as a critical tool for empowering individuals to take control of their health. Through personalized support and education, we’re helping participants not only manage their hypertension, but improve their overall well-being,” said Kukreja.

Participants also take part in two individual consultations, during which their blood pressure is monitored, and any dietary or stress-related factors contributing to their hypertension are discussed.
