Population Health refers to the health status and health outcomes within a specific group of people. Population Health focuses on targeted interventions tailored to specific communities or population groups. This approach includes process of improving clinical health outcomes of a defined group of individuals and a range of determinants, including social, economic, environmental, and behavioral factors that affect the health of these groups.

National Diabetes Prevention Program
The National Diabetes Prevention Program (NDPP) is a CDC-approved year-long lifestyle change program that helps individuals at risk for Type 2 Diabetes prevent or delay the disease through lifestyle changes. Benefits include weight loss, improved health, reduced risk of diabetes, and support from trained Lifestyle Coaches.

Healthy Heart Ambassador Program
The Healthy Heart Ambassador (HHA) Blood Pressure Self-Monitoring Program is a 4-month, CDC-approved initiative designed to empower individuals with hypertension to effectively manage their condition. Participants will learn to control their blood pressure through personalized guidance, nutrition education and ongoing support using self-management techniques and a heart-healthy diet.

Cancer Services Program
The Cancer Services Program is a New York State Cancer Screening Program. It provides breast, cervical and colon (also called colorectal) cancer screenings and follow-up services at no cost to women and men who live in New York State, do not have health insurance, have health insurance with a cost share that makes the cost of screening too high, and meet the program rules for age and income.

Diabetes Self Management Education Program
More than 30 million Americans have diabetes, and every 21 seconds another person is diagnosed with diabetes. Diabetes is a highly complex disease that requires continuous daily care to effectively manage blood glucose levels and help reduce and avoid deadly and costly complications. Diabetes self-management education and support (DSMES) is a vital component of care for all people with diabetes, as well as for people who are at risk for developing diabetes.
St. John’s Episcopal Hospital is accredited by the American Association of Diabetes Educators as a Diabetes Self-Management Education (DSME) Services Program. As a DSME, we offer a 6-week workshop with focus on:
- Healthy Eating
- Being Active
- Diabetes Monitoring
- Taking Medication
- Reducing Risks
- Problem Solving and Healthy Coping

Addressing Food Insecurity On The Peninsula
Population Health Food Distribution
EHS addresses food insecurity in Far Rockaway through a partnership with The Campaign Against Hunger (TCAH). Recognizing the community’s high risk of chronic diseases due to limited healthy food access, EHS and TCAH distribute fresh produce and non-perishables at monthly events:
2nd Thursday, 3rd Wednesday, and 4th Thursday of each month | 12:00 pm – 2:00 pm

Health Bucks Program
EHS also addresses food insecurity in the Rockaways through participation in the Health Bucks Program. This program, supported by the New York City Department of Health and Mental Hygiene, offers $2 coupons that can be redeemed at local farmers markets for fresh fruits and vegetables. Residents using SNAP on an EBT card can receive up to $10 in Health Bucks daily, an incentive designed to encourage healthier food choices and provide access to nutritious options.
