Healthy Hearts Ambassador Blood Pressure Self-Monitoring Program
The Blood Pressure Self-Monitoring Program is a 4-month, CDC-approved initiative designed to empower individuals with hypertension to effectively manage their condition. Participants will learn to control their blood pressure through personalized guidance, nutrition education, and ongoing support using self-management techniques and a heart-healthy diet.

National Diabetes Prevention Program
The National Diabetes Prevention Program (NDPP) is a CDC approved year-long lifestyle change program that helps individuals at risk for Type 2 Diabetes prevent or delay the disease through lifestyle changes. Benefits include weight loss, improved health, reduced risk of diabetes, and support from trained Lifestyle Coaches.

New York State Cancer Services Program
The CSP is a New York State cancer screening program. We provide breast and cervical cancer screenings and follow-up services at no cost to women and men who live in New York State, do not have health insurance, have health insurance with a cost share that makes the cost of screening too high, or meet the program rules for age and income.

Nutrition Workshops
The workshop will provide participants with a sense of ease when it comes to healthy meal planning for themselves and their loved ones. Workshop topics may include healthy meal planning, weight management, management and prevention of specific disease (e.g. Diabetes, High Blood Pressure, Hyperlipidemia), understanding nutrition labels and ingredient lists, lifestyle medicine, food safety, or supermarket tours.

Workshop topics may include, but not limited to health & wellness (i.e. handling health setbacks, receiving a new diagnosis), emotional wellness (i.e. mindfulness, reducing stress, maintaining a position outlook, stress management), personal care/control (i.e. sleep patterns, self-care, body image, self-image, self-esteem, personal hygiene), social networks (i.e. finding supports), safety planning (i.e. impulse control, boundaries, conflict management, identifying supports and risk factors, protective factors), mental health (i.e. depression, anxiety, social anxiety, panic disorders, loneliness), trauma (i.e. grieving, loss, forgiving, coping strategies).
