NYS Children's Health Homes Care Coordination Program
The Care Coordination Program is a team-based approach to case management services with each child and family serviced by a care coordinator. Services provided include:
- Case management services
- Services at home or in the community
- Visits determined by individual needs (depending on level of care)
- Linkage to community services
- Promoting healthy environments, including but not limited to:
- Mental health
- Medical care
- Educational assistance
- Recreational activites
- 24-hour on-call support
- Advocacy meditation
- Psycho-education
- Concrete services, as needed
Who to refer to Care Coordination:
- Children 0 to 21 years of age with a history of
- Mental illness
- ADHD, ODD
- Emotional disturbances
- Trauma
Service area includes the Rockaway Peninsula and all of Queens.
